The Regional Health Assessment (RHA) for 2022 is a project of the RHA Collaborative, a partnership between InterCommunity Health Network CCO, Samaritan Health Services hospitals, and the Lincoln County Health Department – along with the health departments of Benton and Linn counties.
This RHA Collaborative must conduct a health assessment every three to five years to find out what affects people’s physical, behavioral, and social well-being. It is important to gather input from a diverse set of community members to improve our community health planning. Your voice is valued. We need your input and assistance on behalf of our Lincoln County communities.
The Community Health Survey can be accessed online in English and in Spanish.

On request, printed surveys are available in the following languages: English, Spanish, Arabic, Simplified Chinese, Russian, Korean, or Vietnamese. There is a deadline of June 1 to gather the paper survey copies.
