Performance Evaluations

Each supervisor will conduct an annual performance evaluation for his/her employees. The evaluation will be conducted on or before the employee’s anniversary date (as defined in subsection E of this policy). Interim evaluations may be used as needed to document progress. Performance evaluations will be used in decisions about pay increases, efforts to improve work performance, determinations of the need for future training and decisions about promotions or disciplinary action.

Supervisors will meet with the employee to explain and discuss the evaluation. Employees will be given the opportunity to submit a written response in the “employee comments” section of the evaluation form.

Employees are expected to sign the completed evaluation form in order to acknowledge their participation in the evaluation process.

All performance evaluation documents are confidential and will be maintained in the employee’s personnel file. A copy will be provided to the employee for his/her personal records.
